CMS RAC Scenarios For Full and Partial Denials Plus Tips to Tell the Difference

As the nationwide Medicare RAC roll out continues, you need to be keeping an eye out for "full denial" occurrences - cases where your practice will be required to return the full amount of the claim paid, even when the services performed were legitimate.

It's obvious that your RAC auditor will be more aggressive with any audits compared to your Medicare Part B Carrier when it comes down to identifying overpayments due to the fact they are paid on a contingency fee. This means there will be more full-denial situations in which you could potentially lose all payments for a claim.

CMS has hinted at the fact that the deciding factor that you need to be aware of is the place of service in which the services were rendered. When a RAC auditor determines you rendered a service in a setting more acute than what was medically necessary, your claim becomes questionable and the RAC has the right to completely deny the entire claim, leaving you without the opportunity to rebill the claim.

The most common overpayment situation RACs will identify wil result in a partial denial. This happens when you submit a claim with some overpayments and some services properly billed. Rather than loose everything, RAC auditors only ask for the amount of the overpayments to be returned.

For example, if a physician bills a 99214 (E&M code for office visit, established patient) and the RAC determines that a lower level E&M code is more appropriate, the claim will be partially denied. This type of denial is called a partial denial. In this scenario, the overpayment amount is the difference between the payment amount the provider originally received and what should have been paid. The RAC will be paid a contingency fee based on the denied amount.
If you get hit with extensive overpayment demands when your local recovery audit contractor (RAC) shows up in your state, CMS has some advice: appeal immediately.
During the RAC three year demonstration, the majority of appeals were won using two strategies:

New Information/Documentation: Providing new documentation or information will persuade the RAC's thought process by giving them something tangible to focus on. If you think you have additional pertinent information, "present it to the RACs and bring it to the attention of the medical director". It may become apparent that they didn't even receive the original evidence you sent.

Local Coverage Determinations (LCDs): Some RACs failed with LCDs because they change so quickly. You could potentially win an appeal by checking to see if the RAC used an incorrect LCD or if a new LCD exists that the RAC failed to consider.
The key it to save yourself headache by making sure you send all of your documentation you have in support of your claim during the discussion period, before you formally appeal. Entering the discussion period doesn't do anything to your 120 day deadline to file a RAC appeal. That deadline exists no matter how long you spend with your RAC in the discussion period.

Full denials leave you with nothing
If you are paid for services never covered, you obviously should not have received any payment. What happens when you bill the correct services but in the wrong place of service? Unfortunately if the place of service category leads to a change in benefit category, you get hit with a full denial and neither the RAC or CMS can do anything to lessen the burden of the financial loss to your practice.

For example, if an in patient rehabilitation facility claim is billed and the Medicare carrier or RAC reviews the claim and comes to the conclusion that although some of the therapy services were warranted and necessary, such services could have been performed in a less acute setting (i.e. home health agency, outpatient setting, etc.), the claim will be denied in full. In this situation, the rehab facility cannot re-bill the claim and the overpayment is for the entire amount of the original claim billed. The RAC would then receive a contingency payment based on the denied amount.

It's imperative that you monitor these types of denials if your organization is audited by a RAC contractor so that you can determine which ones have the most likely hood of winning an appeal.

Learn Electronic Medical Billing

Medical billing is the process of submitting the claims to insurance companies in order to receive money for the service rendered by a healthcare professional. Before treating and billing a patient, healthcare providers check the eligibility of the intended service with the insurance company electronically. The insurance company has medical directors, who review the claim to check the validity of the payment based on the credentials of the care provider and the necessity of the treatment. A response to the eligibility request is returned by the insurance company through electronic means or more commonly from their website. Proved claims are reimbursed up to certain percentage of the billed amount. Failed claims will be rejected and the healthcare provider will be notified.

Since 1997, all practices which involve patient interaction should pass HIPAA, the Health Insurance Portability and Accountability Act. Under HIPAA, most of the billing claims for the service are required to file via electronic means. HIPAA was enforced in order to prevent fraud and abuse in healthcare delivery and health insurance. While manual filing is time consuming, electronic billing is fast and accurate. With the usage of electronic billing, the rejection and denial has reduced drastically and the percentage of reimbursement has improved.

Electronic medical billing is nothing but the combination of IT and health care sector. Medical practices have been using software to manage their finances for several years. However, the present electronic medical billing programs are well known as medical management practice software. Electronic medical billing software covers a wide range of functions including tracking patient demographics, scheduling appointment, tracking billing information and insurance payments, patient's visits, diagnosis and generating records.

Electronic billing is easy to use, thus helps the staff to improve their productivity. They have also made entire billing much faster .With the electronic billing, any error in the claims are notified to the billing center in minutes, so corrections can be made immediately. Healthcare providers are also satisfied with this system, because electronic billing makes the reimbursement very quick. This software is very friendly to the patients and customers as they give flexibility in scheduling appointment and easy access to the personal information. The cost of electronic claiming is less when compared to the claim submitted on paper. So, insurance carriers encourage electronic medical billing. Electronic billing also helps to eliminate the hassle of posting, printing and sorting the mail.

So, in total, electronic medical billing was a result of combined effort of software companies and healthcare departments to invent a new technology to simplify the process of medical billing.

Electronic medical billing has made the billing procedure much easier and more efficient. The software helps in fast and accurate billing while reduci

Medical billing is a complex procedure of billing and collecting payments for the health care service provided to the patient. To run a profitable medical business, hospitals and other health care facilities should follow an effective way of collecting reimbursement. Electronic medical billing is vital to run a successful practice.

The cash flow in any health care facility depends on fast and accurate billing. This is a tedious process which requires attention to minute details. Accuracy and efficiency are very essential for medical billing. Any error occurred in the data entry will finally end up in denial of the claim. This not only causes loss but also waste of time and energy of the health care professionals. In paper claiming denial is as high as 30%. Electronic medical billing has reduced denials to a greater extent. However electronic billing cannot prevent rejections if not performed by a skilled professional.

HIPAA, health Insurance Portability and Accountability Act requires most of the claims to be done via electronic filing. As soon as a patient enters a health care facility, office personnel takes the personal details such as name, age, sex, address and insurance company of the patient. Before giving any service, the health care processional checks the eligibility of the patient electronically for the planned treatment with the insurance company. This is known as "health care eligibility and benefit response" transaction. Upon treating the patient, medical billers submit a claim form that is customized for each patient to the insurance company and insurance company acknowledges that the claim has been received and will be processed. Once the claim is processed, the payer responds which all claims can be reimbursed or denied with reasoning. With the help of electronic billing, all these procedures are done very fast. So, the doctors will be reimbursed in 10-15 days, where as paper claiming takes months together to get the payment. Electronic medical billing has made the back office job almost paperless and also helps to reduce the number of employees needed in the non-clinical department.

Paper filing requires inventory, postage, phone calls and employee expenses. Since electronic billing is much faster and efficient, the staff requirement is greatly reduced. Electronic billing needs software and computer. When the advantages are considered, spending on computer and software is really worth.

One of the drawbacks of electronic claiming is technical problems. Medical office personnel may not be able to fix the glitches in the software. Electronic medical billing is a good option for those who have already established and have a great number of patients, so that they can afford the electronic medical billing software. Otherwise, the cost of the software itself can make this an unprofitable business.

Medical Billing Made Easier

Electronic medical billing has made the billing procedure much easier and more efficient. The software helps in fast and accurate billing while reducing denials or rejection. It is a cost effective way of increasing cash flow in the medical office.

The improved productivity is directly related to the software used in electronic filing. That means, if well programmed software is used to process the billing, the cash flow is more in the medical facility. As per the government all the electronic medical billing should comply with HIPAA, health insurance portability and accountability act.

Some of the software may provide only partial HIPAA compliance. Any flaw in following HIPAA could lead to complications and penalty. In built compliance prevents any fraud in coding and billing. So before buying any electronic medical billing software, it is better to evaluate the complete HIPAA compliance and other security measures.

This type of software provides a wide range of benefits. While paper claims take around 1-2 months to get paid, electronic claims are paid in 10-15 days. This is possible because any error in the claim can be fixed in few hours, unlike paper claims; where it takes days to notify the error in the claiming.

Patient and customer satisfaction is guaranteed in electronic medical billing. The software is well designed so that it is more user friendly, yet very secure. This gives the flexibility in scheduling appointment and updating any personal information.

With the usage of electronic medical billing, the staff productivity is highly increased. When compared to paper filing, E-billing doesn't require much of man power as it is automated. This software is so easy to use; it helps to process the billing very fast and accurate. E-Billing saves time and money for health care professional by eliminating printing, folding, sealing and mailing the statements. Since electronic medical billing software produces reports and statements by automation, it saves a great deal of time.

One of the disadvantages of electronic medical billing is the cost involved in purchasing the software. Software is available in two forms. First one is completely installed in the health care facility's computer and the other one is a part of the software is in Software Company's data base. Productivity wise both work the same. Technology becomes outdated every few years. So physicians are compelled to buy the new ones in the market.

However, when the advantages of the electronic medical billing are considered, every penny spent on the software makes it really worth.

Best Medical Transcription Courses

Even though formal training is not mandatory to be a medical transcriptionist, having a certified degree or associate degree is very ideal to get a job. There are plenty of courses available to train people in medical transcription. They are available in both the forms; web based training and classroom based courses. However online training courses are more popular because of the convenience and flexibility. It is ideal for those who already have another job, because online training institutes let them learn transcription in their own pace.

Medical transcriptionists convert the verbal medical record dictated by the doctor into text document, which will be included in the patient's permanent medical record. So a thorough understanding of the medical terms, procedures, human anatomy, physiology and abbreviations are very essential. Lack of knowledge in these subjects will interfere with accuracy and interpretation.

Medical transcription courses help students to be fast and accurate. They provide the students with 'real life situations', which helps to work them in a stressful, noisy and busy environment. As learning medical terminology is very important, it is equally important to learn to listen regardless of what is happening in the surroundings, because accuracy and efficiency mainly depends on the importance given to the details.

Medical transcriptionists have to deal with the doctors, surgeons or any other health care professional from all over the world. Hence, it is very essential to understand their dictation no matter what accent and tone they talk. Training courses train the students in this regard, so that they can transcribe in the real life very accurately.

A good knowledge of English grammar, punctuation and spelling is required for a successful transcriptionist. Along with these fast typing, data entry, word processing and computer knowledge are prerequisite for a transcriptionist. Medical transcription courses train these linguistic skills to their students. Transcriptionists should also know about legal and ethical issues surrounding the medical records. Since transcriptionists handle patient's personal and medical records, maintaining patient confidentiality is very essential. Even though medical transcription is a stressful job, it helps to earn a lucrative income.

In the current economy, people are losing job and worrying about how the economy will affect their present job. But economy does not affect health care sector as it affects the other jobs. As far as people need medical support, all the health care department careers are stable. Having a second job is very ideal in this unstable economy. Medical transcription is one among the best careers a person can think of.

Seven Tips For Dealing With Health Insurance Claims

Dealing with medical bills and insurance claims can often become a full-time job. The first step to making sure this doesn't happen to you is to become familiar with all of the details of your coverage. For example, if your insurance has both in-network and out-of-network benefits, you need to understand the difference. When you make an appointment with a new provider, be sure to ask if they are in your network and then check this out yourself on your insurance plan's website or by calling their customer service number. Likewise, many plans require a referral and/or pre-certification for certain visits to specialists or for specific procedures. Don't assume that your doctor's office has checked this out for you. If they say that they will take care of it for you, call your insurance plan yourself anyway and find out if the provider did indeed c all and whether the service will be covered and at what level. Understand the difference between what your provider charges and UCR (usual, customary, and reasonable). Most plans pay benefits based on a percentage of the lower of the charge or UCR and not based on a percentage of the actual charge.
Finally, understand your deductibles and co-insurance responsibility.
Even when you do everything suggested above, there will be times when you don't agree with a provider's bill or how your insurance plan has processed the claim. In that event:

1. Keep detailed written records of every contact you have with the provider or insurance company - dates, times, who you spoke with, what they said, next steps, etc. If you fax something, keep a copy. If you email something, keep a copy. If you mail something, you guessed it, keep a copy.

2. If the customer service person makes you a promise, get it in writing. So if the hospital says they will adjust or write-off a charge, ask for written confirmation, preferably by email. If the claims adjuster says that they will investigate your issue, ask for them to write you a note indicating this, along with a projected timeframe for their response.

3. If your account has wrongly been sent to collections, demand that the provider immediately correct this and provide you with a written explanation that you can send to the credit reporting agencies.

4. Request detailed bills from your provider, and review them carefully. If you don't understand a charge, ask for an explanation. If the explanation doesn't make sense to you, ask to speak to a supervisor. If you believe that a charge is inappropriate (either an error or outright fraud) don't hesitate to challenge it, preferably in writing. If you believe fraud is involved, report it to your state's insurance department and/or federal agencies (in the case of Medicare, for example).

5. If something doesn't make sense to you on a bill or explanation of benefits, question it right away. There are usually grievance or appeal procedures, but these must be done within the timeframes stated in your policy documents.

6. If you are told not to pay a bill while it is being researched or revised, get it in writing! If you agree to a payment plan or a partial payment as "payment in full", get it in writing!
7. Oh, did I mention, get it in writing!

If you don't have the time to handle your medical bills and related insurance yourself, there are professionals available to assist you. Daily money managers and insurance claims advocates are there to take care of these issues for you. You can choose to have an insurance claims advocate review all of your medical bills, or only engage one to assist with sorting out a difficult issue.

Choosing the Right Family Medical Insurance and Medical Insurance Plans

When we settle down and start a family of our own there are a few aspects that we need to give us peace of mind. Good health, stability and financial security are high on our list, so with this in mind there has never been more of an important time than now to take out a family medical insurance plan but with so much choice on the market how can you be sure that the plan you are taking out is the right one for you and for your family? To help you decide take a look at some of the most popular medical insurance plans designed for you and your family;

Fee-for-Service Plans Family Medical Insurance This is not only one of the most traditional types of medical insurance but also one of the most popular among families. With this policy the way it works is your insurer only pays part of your doctor and hospital bills and you pay a monthly fee, which acts as your policies premium. With this policy you are basically given the most choice when it comes to which doctor carries out any treatment you need and the hospital in which you have this treatment carried out. You can use any hospital in the country and there is no need to merely stick with the one doctor as you can change if you wish. What this means for your family is that they are given access to the best care possible at no extra cost to you, giving you complete peace of mind if anything were to happen.

With this policy you are offered two forms of cover; basic or major. The cost of your hospital room and the care you receive are covered by the basic form, along with some services including x-rays and medication. You can also use this cover for the cost of surgery but all of these are generally only on a short term basis. For long term, high cost illness or injury it is a major policy that will benefit you the most.

Point-of-Service Plans, known in short as POS Within this family policy it is possible for primary care doctors in a POS plan to make referrals to other providers in the plan. However it is also possible for you to refer yourself or members of your family outside of the plan as a means of getting coverage. However if you do this you will have to pay extra, known as coinsurance, whereas if your doctor does it then your plan will pay all of your medical bills.

Health Maintenance Organizations, HMO What this policy represents is a prepaid health plan. It involves you paying a monthly premium, so what do you get for this? Well you and your family are provided with comprehensive care, which includes doctor's visits, hospital stays and any emergency care that is received as well as surgery, various tests and x-rays and therapy.

Although with this policy you may have to pay $5 for a doctor's visit or $25 for hospital emergency room treatment, the overall cost of your medical treatment will be a lot lower than, for example, a fee-to-service plan. However your choice of doctors and hospitals will be a lot more restricted.

The above are just a few examples of the medical insurance policies that are available to you and your family. Just ensure that before you decide on the healthcare plan you are going to take out that you weigh up what each of the polices has to offer you in order to get the most from the policy that you do decide to take out.

The Medical Billing Specialist

Medical billing specialist plays an important behind-the-scene role in the health care system. Medical specialist is someone who does billing work for a health care facility. They help to speed up the process of payment. This type of billing is considered to be one of the fastest growing careers in the medical sector. These trainings usually range from one year to two years. Anything less is not so recommended as hospitals and other organizations don't prefer them.

Medical specialists earn a respectable salary. But wage mainly depends on the years of experience in this field. Medical billers usually work in an office setting, Even though they work for health care facilities, they don't work where the patients are being seen. They usually work 8 hours a day, but sometimes this job demands overtime work. Medical billing specialists have been able to work from home as independent consultants. Medical Billing Services Review

Financial strength of a health care provider is directly associated with the timely billing and reimbursement. Medical billing is the process by which health care professionals collect fees from the insurance company for the service provided to the patients. Health care professionals are so busy taking care of their patients; they have no time to worry about their billing. Finding a medical service provider is one of the most important tasks for a health care provider. Billing service providers help them by timely submission of the claims to the insurance company by taking a fee. They relieve the doctors from following up with insurance company, thereby helping them to concentrate more on practicing medicine. Hospitals do not need billing employees anymore. This has reduced the non-clinical staff requirements in the hospital.

For a long time, billing was entirely done on papers. But, paper documentation is a tedious and time consuming process. However, with the advent of software technology, billing has become much easier as medical service providers do electronic claiming. This helps them in fast and accurate claiming. Medical service provider's duty includes verifying the insurance coverage, determining the eligibility, submitting the claim, following up the claims and contacting the patients and the insurance company to ensure the highest possible reimbursement. Health care providers who used quality medical billing service providers have seen improved performance in multiple dimensions, such as increased and fast collections, decreased denial of insurance and increased staff productivity.

Medical billers not only process the claims but also analyze the patient records to prepare customized report. They also advise the doctor on revised fee structure, new coding and ways to improve the medical practice as a business. Since medical biller's duties involve complex areas such as bill collection, release of patient information and patient confidentiality, extreme care should be practiced while handling this area of billing service. A good sense of responsibility and consciousness of patient confidentiality is very essential for a medical biller.

A medical biller is expected to know good keyboarding, medical office procedures, medical insurance, medical terminologies, health care law and ethics and basic coding.

So, in a nutshell, medical billing service comes into picture each time a patient visits a health care provider. After each patient visits, medical biller reviews the bill attached to the patient's record to establish which service was provided. Next, they check the insurance coverage and produce a customized invoice to the insurance company for the reimbursement. Medical billing service comes in handy to run a successful medical practice in a convenient way.

Med Billing Services

Doctors and other health care providers are facing great deal of difficulties in getting the reimbursement for the service provided. Medical billing takes considerable time and effort and health care providers cannot accommodate it. To eliminate these problems, health care providers usually outsource their billing work to reputable and reliable medical billing service providers.

Medical billing is the process of submitting the claim to the insurance company in order to receive the payment for the service provided. Accurate and timely submission of the claim is required to make smooth functioning business. Medical billing and coding go hand in hand. While billing involves submission of claims, coding assigns special codes for the treatment and service rendered. Any discrepancies in these two can end up in rejection or denial of the claim.

Medical services are a growing group of business. They provide billing services and ease the back office work of the physician. Time frame is very important in billing. If the claiming is done after the window is closed, it is very difficult to get the reimbursement. Medical companies have experienced, skilled medical billers, who can do medical billing accurately and efficiently. They speed up the process of claiming and help to maximize the reimbursement. They do so by using technologically advanced billing system such as EMR (Electronic medical record) and EPM (Electronic practice management).

Hiring a medical services is one of the big decisions in a physician's business. Finding a reputable reliable service provider is very essential. They should be aware of the fraud companies, who claim to give a quality service. Privacy is the utmost important topic in the medical industry. Anyone who comes across the patient's records has to maintain the patient confidentiality. There are many rules and regulations to be followed by the medical services. If the billing service does not abide by the rule, it can cost a fortune to the physician. So, it is always better to do research before hiring a medical billing service and make sure that they provide the service needed. The benefits of hiring a medical service are: saves time, faster billing, minimal rejection, maximum reimbursement and less number of back office staff.

While the primary job of a physician is to give patient care, financial well being of the practice enables them to provide quality service to the patients. Medical billing service helps the health care professionals to achieve that.

Medical Billing is very important o hospital, clinics and doctors, so if you want a good career change try looking at some medical billing and coding schools for more information on becoming certified.

Medical Billing Salary Range

Although medical billing career may not earn a very high pay like other medical related jobs, medical billers do bring home a salary that is enough to live a fairly comfortable life.

Medical billing salary is influenced by lots of factors. The training gained in the billing field accounts for the major part. The person who completed a relevant course in the medical field, gains special skills and knowledge and this is a clear indication that he/she is putting extra efforts in order to perform a good job as a medical billing specialist. Depending on the education qualification, the hourly rate varies from $12-$15. Another most important factor that affects billing pay is how long someone has worked in the field. Medical specialist with experience of 1 year earns around $12 per hour. Those who have more experience in billing earn up to $16 per hour. However, the geographic location also plays a role in pay scale. For instance, areas where cost of living is high, the pay will be more. Billers who work in New York City, Houston, Chicago and California are able to pull a good amount of salary. Work locations such as hospital, billing company or private practice will also affect the salary. Like all the other jobs, skilled and experienced billers enjoy a good salary. Their earnings are somewhere around $14-$16 per hour. A skilled biller should be self-motivated, have good follow-up and should be able to give importance to the minute details in a patient's file. Medical professionals who are working from home usually charge based on hours of work. Hourly wage ranges from $10-$20, depending on the certification and experience they have.

There are many companies who do false promise on the salaries and make the candidates to shell out a lot of hard earned money to get a job. So prior signing any document or commitment, a detail research of the company is essential. It will save a lot of money and peace of mind.

Medical Billing Company

Health care professionals are so occupied in patient's treatment that they do not have time or energy to do their billing job. But they cannot neglect this sector of their practice, as cash turnover is the key to success of a practice. Cash will flow smoothly only if the billing is done accurately and on time. Some companies have come up with a solution to health care professional's billing problem. These organizations are called medical billing companies.

Medical billing is a meticulous job which needs accuracy and attention to details. Billing companies employee skilled medical billing professionals to do the billing related jobs. So, health care facilities are never at risk. Medical transaction companies are located world wide. When a billing job is outsourced to a company located in a developing country, it is a very cost effective business for the health care facility.

One of the main advantage of outsourcing the billing job is time saving. Billing is a time consuming procedure. Since this is done entirely by a third party, doctors are able to concentrate on their main job; patient's treatment. Out sourcing has reduced the burden on the back office staff. So, the need of resources has greatly reduced. Medical billing companies process the claim using electronic billing software. When software is used in billing, the errors are reduced and claims are processed in a very fast and efficient manner. This helps in quick turn around of the cash. Most of the companies use software that is HIPAA compliant and billing is done using state-of-the-art network. This is highly essential to prevent the fraud happening in the billing.

There are many companies out there to provide billing service. The main goal of these companies is to maximize the reimbursement in a speedy timely manner. So, hiring a right medical billing company will significantly improve the profit of the health care facility.

Medical billing companies charge their clients for the service they provide. They either take a percentage of the reimbursed amount or they charge for the number of claims they processed. In either way, it is a very cost effective to the doctors. This helps in saving money by avoiding staff payment and benefits, software purchase, printing, posting and other inventories required in billing.

So, handing over the billing procedures to the medical billing companies significantly improves the clinical, financial and administrative performance of the health care facility.

Insurance Billing Software Review

The need to increase the productivity at a reduced cost while providing the best service to the patient is the reason why health care professionals go for insurance billing software. Doctors have been using the software to manage their finances for quite a long time. But software is doing wonders in the health care system. Today's software gives a comprehensive solution to the health care professional's back office work. This is well known as medical practice management software. The software does everything from scheduling the appointment of the patient to the collection of reimbursement.

Most of the available billing software products in the market are HIPAA compliant. However, some of them may be only partly HIPAA compliant. When health facilities use complete HIPAA compliant software, there is no scope for the violation of the regulation. So, physicians don't have to worry about the penalties of violation of HIPAA.

Insurance billing software reports any past due payments to both patients and insurance companies and notifies the employees about any balance or co-payment to be paid by the patient. So, medical office employees are more informed about patient's payments and help them to collect the co-payment, which if not collected on time can lead to complications later.

Automation of generating the reports and statements has reduced the requirement of back office personnel in the health care facility. Entering the data and retrieving them are so much convenient with the use of software; it saves time and makes it more efficient. Insurance billing software helps to streamline the process of billing and takes of the pressure off employees. So, the productivity and efficiency of the staff has improved to a greater extent.

Insurance billing software can be either client server or application service provider. Client server requires a front end payment. This has to be installed in the medical office computer and is linked to only the local network. The disadvantage of this is that, any glitch in the software makes it necessary to hire some IT staff, as health care professionals cannot fix it. Where as, in application service provider, the server is located in the software companies data base. The software company manages the technology, recovery and other maintenance. This kind of software is usually paid on monthly basis.

Whether it is client server or application service provider, the health care professionals need to have some kind of training to operate this. Most of the medical billing courses train their students in this aspect. Insurance billing software is a basic requirement for a well geared organization. It is a cost effective way to improve the productivity.

Correct Coding Initiative Works Towards Averting Improper Payment

The National Correct Coding Initiative (NCCI) was developed by the Centers for Medicare and Medicaid Services (CMS) to promote national correct medical coding methodologies and to control inappropriate coding leading to incorrect payment. The CMS (then the Health Care Financing Administration) had implemented the first round of correct coding edits in the year 1996.

These code edits are pairs of Current Procedural Terminology (CPT) or Healthcare Common Procedure Coding System (HCPCS) Level II codes that aren't payable separately except in certain circumstances. These edits are applied to services billed by the same provider for the same beneficiary on the same date of service.

NCCI comprises two tables of edits - Column 1 /Column 2 Correct Coding Edits table and Mutually Exclusive Edits table. Previously Column 1 /Column 2 edits were known as Comprehensive/Component edits. Column 1 code includes the Column 2 code; hence Column 2 should not be billed separately. The second category, the Mutually Exclusive Edits identify coding combinations comprising two procedures that cannot be performed together based on the CPT code definition, anatomic considerations or standard of medical practice.

The CMS updates the National Correct Coding Initiative Coding Policy Manual for Medicare Services (Coding Policy Manual) annually. You can avail the Correct Coding Initiative Edits Manual in two ways. Firstly, you can go to the CMS Website .hhs.gov / NationalCorrectCodInitEd/. The Website contains a list of the CPT edits and is free for downloading. Secondly, you can avail it by purchasing the manual or sections of the manual from the National Technical Information services (NTIS) .

Medical Billing - Are You Prepared For the Red Flags Rule?

Red Flags Rule is instituted by the FTC and it is designed to protect against identity theft. The rule will take effect on November 1, 2009. It sets out how physicians offices, clinics and hospitals assess and identify medical identity theft. This occurs when a person uses another person's personal information (Name, Social Security Number or insurance information) without the victim's consent to obtain medical services. As defined by the FTC, "Red Flags" are suspicious patterns or practices that indicate the possibility of identity theft.

Physicians and medical practices are covered under the rule if it is a "creditor" that offers or maintain "covered account". FTC classifies physicians and medical practices as "creditor" because they extend credits to patients when they bill them and do not collect at the time of service. Patient billing account is considered a "covered account" because it permits multiple payments and it carries a reasonably foreseeable risk to patients.

Four steps to developing a Red Flag compliant program:
1. Identify relevant red flags in your practice. Your program must include policies and procedures to identify the red flags of identity theft 2. Detect red flags: Set up procedures to detect those red flags in your daily operations. 3. Prevent and Mitigate identity theft: Respond appropriately to prevent and mitigate. 4. Update your program and keep it current as the risks of identity theft change rapidly.

The FTC guidelines offer a detailed description of each step as well as providing some examples.

Steps physicians and medical practices can do to prevent a compromise of patient record:
• Check patients photo IDs and require a second identification prior to providing medical services.
• Train staff to detect fraud and misuse.
• Monitor who is accessing patient files and records.
• Look for patterns or suspicious activities in patient accounts

• If you detect a fraud, report incident to a law enforcement agency and lockup patient account.
After the Red Flags Rule take effect on November 1 of this year, physicians and medical practices must keep their program updated and to guard against latest threats. Physicians who fail to comply could face a fine of up to $2,500 per identity theft incident. Physicians may even face a lawsuits as well.

Resources to learn more about Red Flags Rule:
The AMA-American Medical Association has prepared a guidance document, along with sample policies to help physicians incorporate a simple identity theft prevention and detection program into their existing compliance and HIPAA security and privacy policies. You can visit The AMA website to access these documents to access these documents.

In cases where an on the job related accident prevent s the worker from doing his or her job the workers compensation fund helps to pay for the loss of wages that are incurred because a person cannot physically do the work that they once did.

Tragically the number of cases that are examined and claimed for are related to hearing or eyesight loss as industrial complexes use heavy machinery to produce there finished work. Even with proper protection the deafening sounds of the motors and engines that workers are exposed to can cause permanent hearing loss through prolonged exposure.

Other on the job accidents may result in permanent or temporary injuries that put the victims of such industrial accidents in the hospital for a period of time. What the Insurance Companies Don't Want Your Business to Know About Workers Compensation

Do you know how many states in the United States allow a business to go without mandatory workers compensation? If you said one, you're right - Texas is it. Even with that said, most businesses in Texas carry Workers' Comp to be sure they are protecting the financial stability of their business in the best possible manner.

If a business pays over $5000 in insurance premium for workers' compensation, an experience modifier is applied after 3 years of continuous coverage. This modifier is a reflection of several things; insurance premiums paid by a business, the size of the claims paid from injuries, frequency of claims, expected amount of claims based on the classification and a host of other calculations. These modifiers in Texas are calculated by the National Council on Compensation Insurance, NCCI.

All of this has a direct effect on what a business pays for workers' compensation. For instance, a modifier of 1.0 is average, .80 is 20% better than average and a 1.20 is 20% higher than average. This is directly related to what you pay for workers' compensation, and in some cases insurance companies add discounts if you are under 1.0. What this illustrates is the importance of keeping this number down.

When a claim is filed, and left open for future payments, insurance companies set a reserve on that claim which is their guess of the ultimate settlement. That allows them to budget and know what to expect on that particular claim. In most every case, that reserve is set high - higher than what the final payment is.

Modifiers are calculated exactly 6 months from the original inception date of the workers compensation policy. So, for a WC policy from 1/1/09 - 1/1/10, the 2011 modifier is calculated on 7/1/09 (exactly 6 months into the policy term). If a claim happens on 6/1/09 and a reserve is set higher than it should be, the new modifier is going to reflect the inflated numbers resulting from that particular claim. That means next years' modifier will be higher than it should be and the business pays an inflated cost throughout all of the next year.

And of course, Insurance carriers are fickle. The higher the modification on a policy, the less they want to write it. Even though they make more money with a high mod, many carriers have a very hard time writing a business with a modifier higher than 1.10. And you know what this means; higher premiums. On the flip side, a business with a low mod is very desirable and can get additional discounts to further lower their premium.

What can a business do? Loss Control and Monitoring each claim individually is the key. Watching claim reserves, data transferred between the insurance company and NCCI, proper worker classification (to name a few) is what has to happen. No matter what you pay for workers' compensation, if you have an experience modifier it needs to be monitored diligently.
Or, of course' you can trust your insurance carrier to reduce reserves at the appropriate time - which is not going to happen. Why would an insurance company care ? If the modifier is higher, they make more money for the exact same exposure.

An individual business can play a big role in keeping their workplace safe, but the monitoring is usually something that has to be outsourced. Find a good commercial Insurance Agency you trust and allow them to continually monitor your policy and report the activity back in detail, outlining the amount of reserve reductions. This can save a business thousands of dollars, but the average is 27%.

The cost of this service can range anywhere from 2% (of the workers' compensation premium) down to free depending on the size of your insurance premiums and if the Agency also handles your other insurance policies. Many times Agencies provide this monitoring as a valued added service if they handle all your other policies.

Over a period of just a few years you can easily be paying 27% more than you should for workers compensation if it's not closely monitored. Keeping this modifier down is an important part of controlling the overall costs of businesses insurance. Control your modifier and you control your bottom line.

Neck and Shoulder Injuries on the Job

Neck and Shoulder Injuries

Many employers take great pains to stress the importance of workplace safety to all employees in the workplace. While most awareness efforts tend to focus on slip-and-fall injuries, back injuries caused by heavy lifting, and injuries caused by falling items, there are many other types of injuries that can occur in the workplace.

Workers commonly report suffering from neck and shoulder injuries while on the job. Such injuries can be caused by major incidents or relatively minor motions. Neck and shoulder injuries can seriously affect an individual's ability to work and perform the required duties of their job.

Serious neck injuries can be caused by workplace collisions, falling objects, or accidents that cause severe whiplash. Workers who are required to operate machinery or vehicles as part of their duties are at risk of whiplash injuries on the job. People may also suffer serious neck injuries if they fall from elevation while at work.

Less serious neck injuries can be caused by repetitive motions in the workplace. Employees who are required to move their heads constantly may experience neck pain and soreness as a result. Although neck soreness may not seem serious at first, repetitive motion that causes pain may ultimately prove to be very harmful to the long term health of the individual.

Shoulder injuries are also common in the workplace. Employees who are required to lift and carry heavy objects may experience pain and soreness in their shoulders as a result. Accidents in the workplace involving collisions or slip-and-fall accidents can also damage an individual's shoulders, so it is important to be careful at all times on the jobsite.

People who experience neck and shoulder pain may find it difficult to work. Workers experiencing chronic pain often have issues that limit the range of motion and make concentration difficult. If you are injured as a result of a workplace accident or pain stemming from repetitive motion, you may be eligible for workers' compensation benefits.

Worker's compensation exists to help injured employees pay for medical bills, expenses, and other financial needs as they recover from the injury. If you have been wrongly denied worker's compensation benefits, consult an experienced employment compensation attorney to discuss your legal options. For more information on neck and shoulder injuries, consult the Philadelphia neck and shoulder injury lawyers of Lowenthal & Abrams, P.C.

Avoiding Workmen's Compensation Claims

Workmen's Compensation Insurance claims can be quite costly. Workers comp rates have skyrocketed over the last 10 years and even in the last couple years things have really gotten out of control. Generally this is because of lawsuits, and the hyperbolic climbing costs of health care.

One small injury, which would normally have cost very little, can run into the thousands of dollars very quickly. Even an employee that merely cuts their fingers and goes into urgent care can come out with a $3500 bill. And they can often claim, along with a doctor's note that they can't work for a couple weeks. Of course, you still have to pay them.

The doctors also are very fearful that they tell someone they can go back to work and they get an infection on their cut finger that they might be sued as well. So they take extra percussions that are usually way over the top.

You should avoid worker's compensation claims all costs, and that means training your employees and having them read the manual and not assuming that they are smart enough to not hurt themselves. Further, before you hire an employee you should check with their previous employers and make sure they did not file any worker's compensation claims with them.

Chances are if they did, they were a problem employee, and they sued their previous employer for a lot of money on an injury that was not even real. There are ways to protect yourself and perhaps you should get with a worker's compensation attorney that works on the employer side of things and ask them what you can do to minimize your risk.

Also check with your insurance agent and if you hadn't had any worker's comp claims in the last year ask for a reduction in premiums. It may not lower your rates because rates are increasing in price, but you may not have to deal with the price increase. Please consider all this.

Ask Workmen's Compensation Insurance Companies For Discounts For Employee Safety Training

If you own a small business you should ask your insurance broker to contact their underwriter for worker's comp, especially if you hadn't have many claims in the last year. You should ask them for a discount.

Whereas, you may not get a discount or reduction in worker's compensation premiums, you may not get the price increases that all the other businesses in your industry are getting. Further, while you are at it you should be asking your insurance broker for discounts on premiums due to your employee safety training.

Even if the underwriter says they cannot do this for you, it will open a dialogue between your commercial business insurance broker and the underwriter and they may come up with ideas to help you reduce your workers compensation premiums. It doesn't hurt to ask, and sometimes the insurance companies have safety workplace training programs themselves, many of these are voluntarily available to business owners. You should take advantage of this if you can.

This due diligence will also help you in case of a lawsuit, because you can prove, and document that you are progressive in working towards a strong workplace safety environment for all your employees. You never know, when an employee might try to file a fake claim, raising your premiums, and then suing you in court.

Being able to prove that you go beyond the normal OSHA standards in training your employees with all the safety issues may end up saving your business in case of a top-notch litigating lawyer trying to sue your business into oblivion. Please consider all this.

Proper Training Prevents Workmen's Compensation Claims

There are a tremendous number of OSHA laws that govern workplace safety, and there are things that workers have a right to know. They have a right to know which chemicals are being used in the workplace that might be adverse to their health, and they have a right to know which tools they might be using that could injure them.
Still, sometimes it make sense to go beyond the OSHA laws for workplace safety and consider proper training to prevent workers compensation claims. As you are probably aware employers are charged more money each year depending on the number that claims they had in the previous year.
If you go for two or three years with no claims chances on your workers compensation insurance, you should have your premiums go down, or at least stay level while other people in the industry continue to get price increases.
When I say proper training prevents workmen's compensation claims, what I mean is that if you show training videos of how to do the operations of your business with safety in mind you will prevent injuries. Plus, in the event of a big lawsuit if you can demonstrate and document that you have proper training, and chances are an employee will not be able to sue your business or prevail in an intense lawsuit.
More than one small businessperson in your area has lost their business to a lawsuit due to an employee getting hurt on the job. Most of the employees hurt on the job, well, it's actually the employees fault but the courts and juries don't always see it this way. One bad accident can cost you your entire business, so get proper insurance and do the safety training to the best of your ability. Please consider all this.

Denial of Worker's Compensation Claims

If an employee is injured in an accident at work, he or she may be eligible for worker's compensation benefits. Most employers offer worker's compensation coverage for their employees to help them pay for medical procedures, rehabilitation, lost wages, and medication. Although many work-related injuries may qualify for assistance through worker's compensation programs, there are certain circumstances in which the worker's compensation claim may be denied.
If an employer chooses to deny a worker's compensation claim, the victim may appeal the decision or may decide to take their claim to court. Some cases may include eyewitness statements and proof that the person was injured due to faulty machinery, hazardous conditions, accidents caused by the requirements of the job, and other valid claims. The court may find that the person is not eligible for compensation if he or she behaved recklessly or did not follow office safety rules and regulations and was injured as a result.

Compensation claims may be legally denied if the employee attempts to collect on an injury that was self-inflicted. Although this action may seem outrageous, some individuals may attempt to cheat the system by faking or deliberately setting up an injury to collect against their employer. If an employee's injury is revealed to be self-inflicted, the employer may have a justifiable reason to deny the compensation claim.

If an employee is found to have been under the influence of alcohol and drugs, the employer may have valid reason to deny a claim against them. A vast majority of employers have rules against working while under the influence, and it is reasonable to expect employees to be of sound mind and body in the workplace. A worker who was intoxicated at work cannot reasonably claim that the accident was the fault of the employer if he or she was unable to prove proper cognitive ability and physical dexterity.

If the worker was injured on the job while breaking the law, he or she may face criminal charges in addition to denial of any claims for compensation. The employer may find reason to pursue additional charges against the person even after the denial has been lodged.

It is important for individuals to understand that worker's compensation claims may not be immediately approved, especially if he or she was injured while behaving in a reckless, negligent, or illegal manner.
